This position evaluates social and emotional needs of residents to assure overall wellbeing, interacts with all departments to ensure quality of care for the residents, assists with Medicare admissions/ discharges, and completes documentation including but not limited to MDS, RAPS, assessments, notes, discharge planning, and care plans.
The successful candidate should have a Bachelor’s degree in social work or human services and 1 to 2 years’ experience working in a geriatric setting.
